Financial Policy

I authorize and give consent to perform the dental services agreed between doctor and patient and or parent or guardian to be necessary or advisable including the use of local anesthesia and other medications as indicated.

We appreciate you choosing our office for your dental care needs. At Laser Dentistry of Coeur d’ Alene, we value our relationship with you and your family and would like to offer the following as our payment policy.

* In case of insurance, we will be happy to help you maximize benefits available under your policy. However, please realize that the relationship between you, the insured, and your insurance company. In the event of duplicate payments, your account will be reimbursed. In some cases, our office will not be able to estimate benefits from your secondary coverage.

Our office will gladly file claims with your insurance, however if your insurance has not paid their expected portion after 90 days and 2 claims submissions, the remaining balance will become due and payable by the insured or guarantor immediately.

For your convenience, we accept cash, checks, VISA, MasterCard, Discover, American Express and ATM Cards. We also have special financing options available through CareCredit.

Please note that estimated patient portions and deductibles are due at the time of services. Only if payment arrangements have been made and approved will we proceed with treatment.

We ask that cancellations or changes to appointments be made at least 48 hours in advance. It is our policy to charge $50.00 for a broken appointment. Repeated broken appointments may result in dismissal from our practice.

A service fee of $35.00 will be applied to any returned or cancelled checks. Any unpaid balances over 30 days will be assessed a finance charge of 18% APR. Any balances unpaid after 90 days will be assigned to a collection agency for handling and will be assessed a collection fee of $65.00 plus any other collection costs/fees incurred while attempting to collect a debt.
